Required Forms
Once you accept the invitation to the national tournament, a Science Olympiad staff member will contact you with the following forms:
Medical Release Form - One per student, signed by parent/guardian, and kept in the possession of team coach; form must accompany the student to any medical intervention. At check-in, coaches must certify that they possess the forms for all students.
Consolidated Permission Form - (covers parental permission, acknowledgement of medical forms, and media release): One per student, signed by parent/guardian, and submitted electronically to unlso25@unl.edu by May 18.
Final Team Roster - The final list of students who will compete in the national tournament, in addition to alternates. The form should be submitted electronically to unlso25@unl.edu by May 18.
Coaches/Chaperone Media Release Form - Read and signed by coaches and chaperones and submitted electronically to unlso25@unl.edu by May 18.

ILLNESS OR INJURY
In case of illness or injury, contact the front desk. If you must leave a camp or conference due to illness or injury, please see your counselor or coach for directions. In the event of a serious or life-threatening injury always call 911 first.
FIRE
In case of a fire, the alarm will sound continuously. You should:
- Walk rapidly to the nearest stairwell exit on your floor. Do not use the elevators!
- Leave the building and stay clear of all fire equipment, firefighters, and driveways.
- Follow the directions from staff members and firefighters.
- Enter the building only when instructed to do so by the hall staff or emergency personnel.
TORNADO
A tornado watch indicates that weather conditions are favorable for a tornado. If the National Weather Service issues a tornado watch for Lancaster County, we will post signs in the main lobby of the hall. Please be ready to move to the basement via the stairwell.
A tornado warning means that a tornado has been sighted in the immediate area. If the National Weather Service issues a tornado warning, the residence hall emergency alarm system will sound.
You may also hear Civil Defense sirens outside. You should:
- Walk rapidly to the nearest stairwell exit on the floor and go immediately to the basement of the hall.
- Follow the directions from staff members.
- Stay in the basement area until staff members indicate that the tornado warning is over.
Failure to comply with safety procedures or staff instructions will result in removal from the residence halls.
