Required Forms
Once you accept the invitation to the national tournament, a Science Olympiad staff member will contact you with the following forms:
Medical Release Form - One per student, signed by parent/guardian, and kept in the possession of team coach; form must accompany the student to any medical intervention. At check-in, coaches must certify that they possess the forms for all students.
Consolidated Permission Form - (covers parental permission, acknowledgement of medical forms, and media release): One per student, signed by parent/guardian, and submitted electronically by May 18.
Final Team Roster - The final list of students who will compete in the national tournament, in addition to alternates. The form should be submitted electronically by May 18.
Coaches/Chaperone Media Release Form - Read and signed by coaches and chaperones and submitted electronically by May 18.
